I have tried this earlier but did not realize that there are 2 instances of the header removal, thus I only removed the first.

Hi Daniel,
You can modify the headers any way that you want in the exim routers
section of the exim configuration file (/etc/exim/exim.conf). �To always
include the X-Spam-Report header simply locate the "headers_remove"
statements in the routers section of exim.conf and then delete all
instances of "X-Spam_Report" from the "headers_remove" statements for
each respective router. �Subsequently, the X-Spam_Report should be
included in all emails.
